CPS Program
The purpose of the peer support program is to link individuals with a peer mentor who serves as a role model and demonstrates that recovery is possible.
Peer mentors serve as role models in recovery who encourage individuals to seek treatment, when necessary, maintain wellness, take an active role in their own treatment plans and attend self-help groups (AA, NA, and other consumer run support groups.)
Peer support services are available only to individuals diagnosed with serious and persistent mental illness (DSM-V). Referral must be signed by a physician. Call to request a referral.

Our Certified Peer Support Program focuses on cooperation and shared responsibility. We help our peers set and reach our goals aimed at their recovery. We provide advocacy, empowerment and opportunity for personal growth and recovery.
Our mentors have been on a personal recovery journey, and this allows them to share their experiences and recovery tools with our peers. The Peer/Mentor relationship is built on equality and trust, brought about by establishing rapport and agreement on what the peer needs. We link peers to services such as housing, transportation, medical, education, and crises as well as other social, spiritual and recovery supports.
We operate within a supportive and cooperative network with other agencies like doctors, therapists, and blended case managers. Our peers are at the center of that network, driving the effective improvement of their own lives.

Program Requirements
Must be resident of Clearfield, Jefferson, or Clarion counties, ages 14 and above, have a mental health diagnosis with a functional diagnosis with a functional impairment/skill deficit. If you have any questions regarding requirements, please call our Clearfield office at (814) 765-0302.

When consent is given and information is released, the information is confidential and not be shared with anyone else.
